How to use this worksheet

  1. Copy the existing plan and dates accurately.
  2. Describe headache frequency, impact and unwanted effects in plain language.
  3. Write the decision or question you want to discuss at follow-up.

Use comparable date ranges and note missing diary days. Improvement or worsening can have several explanations. Your clinician should interpret effectiveness and unwanted effects with you.
