Alcohol and migraine: red wine, timing, and the evidence
Red wine is the most famous migraine trigger in the world — and one of the least proven. Here's what research actually supports, why timing matters more than the drink, and how to test your own sensitivity without giving up everything at once.
A famous trigger with a thin file
In surveys, roughly a third of people with migraine name alcohol as a trigger, and many avoid it entirely just in case. Yet when researchers try to pin it down under controlled conditions, the results are inconsistent: some diary studies find only a weak link between drinking and next-day attacks, and provocation studies — giving people red wine and watching what happens — have been small and contradictory. That doesn't mean alcohol is innocent. It means the effect is real for a subgroup, variable in dose and timing, and hopelessly entangled with everything else that happens on an evening out.
Why red wine gets the blame
Red wine's reputation likely comes from its chemistry and its company. Beyond ethanol, wine carries congeners — histamine, tannins, phenolic compounds and other byproducts of fermentation that give dark drinks their color and depth. Red wine contains far more histamine than white; darker spirits carry more congeners than clear ones. Each of these has been proposed as the true trigger, and none has been convincingly convicted. Sulfites, the preservative most often accused by drinkers, make a poor suspect: white wine usually contains more of them than red, and sulfite-rich foods like dried apricots almost never appear on trigger lists. As the American Migraine Foundation notes, the honest summary is that any alcoholic drink can trigger an attack in a susceptible person, and which drink is worst varies from one person to the next.
The two timing windows
Timing is the most useful clue you have. Alcohol-related attacks arrive in two distinct patterns:
- The immediate attack — beginning within about thirty minutes to three hours of drinking, sometimes after a single glass. This rapid response, often to a small dose, is the pattern most suggestive of genuine alcohol sensitivity rather than a hangover.
- The delayed attack — the next morning, in hangover territory. Here the picture is muddier: ethanol's own after-effects, dehydration, disrupted sleep and a late night all pile up. In someone with migraine, though, this often isn't an ordinary hangover — it's a full attack, with the nausea, throbbing and light sensitivity turned up far beyond what the amount drunk would explain.
Dehydration deserves its own mention: alcohol suppresses the hormone that tells your kidneys to retain water, so a night of drinking leaves you genuinely depleted — and dehydration is itself a migraine trigger. Drinking water alongside alcohol doesn't neutralize a true sensitivity, but it removes one co-conspirator from the lineup.
An evening with wine is rarely just wine. It's also a late bedtime, a restaurant meal with aged cheese, a skipped workout, maybe a stressful week ending. Before sentencing red wine to exile, check whether your attacks follow the drink — or the whole evening.
Testing it: pours versus attacks
Because alcohol is an occasional, discrete event — unlike stress or weather — it's one of the easiest triggers to evaluate, if you record honestly:
- Log what and how much you drank, that evening. Type matters: if red wine triggers you but whisky doesn't, that's the pattern worth finding.
- Log every attack promptly, with its start time. The immediate-versus-next-morning distinction is the whole diagnostic, and it evaporates if you log a day late. Dim takes three taps to log an attack mid-evening, and captures sleep and weather on its own so those confounders are in the record too.
- Compare across occasions. One bad night proves nothing. Five or six drinking occasions with times and outcomes tell you whether attacks follow the drink, a specific drink, or only the drink plus a short night.
What to do with the answer
If a specific drink reliably precedes attacks, the fix is obvious and yours to choose. If the pattern only appears with the whole late-night package, you may have more room than you feared — smaller amounts, earlier evenings, water alongside. And if attacks strike after tiny amounts of any alcohol, mention it to your doctor; a diary showing dose, drink and timing makes that conversation short and productive.
