How to use this worksheet
- Write the symptom rather than guessing its cause.
- Record onset, end and whether the time is approximate.
- Note whether the same symptom has happened before.
Describe whether each symptom is familiar or new. Sudden severe headache or new weakness, confusion or difficulty speaking needs urgent medical attention, not a completed worksheet.
