A single end time can hide a slow return to normal. Use this timeline to record observations before pain, during the headache and after pain settles without assuming that every stage is present.

How to use this worksheet
- Start with the earliest event you can recall reliably.
- Add medication and symptom changes in chronological order.
- Record pain ending and return to usual activity separately.
Keep symptoms and timestamps separate from explanations. If the timing is uncertain, say so. New or unusual symptoms deserve medical advice even if they resemble a previous entry.
