Migraine pain scale: make diary ratings consistent
A pain rating is most useful when you use the same scale each time. This worksheet pairs your own rating with an everyday activity so the number has context.
Your worksheet
Your entries stay in this tab. Nothing you type is sent to Dim or saved automatically. Download before closing or refreshing. Avoid names or other identifiers if you plan to share the file.
How to use this worksheet
- Write down the scale you chose, such as 0–10.
- Rate at a recorded time rather than rewriting the whole day from memory.
- Add a concrete activity affected and any change later.
There is no universal conversion between someone's rating and yours. Use your diary's scale consistently and describe what you could or could not do. This is not a validated disability score.

This is an organizational worksheet, not a diagnostic test or a treatment recommendation. Discuss new, changing or worrying symptoms with a clinician. Do not wait to complete a diary before seeking care.
