How to use this worksheet

  1. Write the reporting dates and how many days have usable records.
  2. Summarize headache days and medication-use days separately.
  3. Add the main change and one question for your clinician.

A month with better logging may appear worse than a month with missing records. Label the date range and compare like with like before drawing conclusions about change.
